Use Form 200 to request relief from liability for tax, plus related penalties and interest, that you think only your spouse (or former spouse) should pay. This includes relief for separation of liability, innocent spouse relief, or equitable relief.
Form is used by a taxpayer to compute a tax credit, under Arizona's Claim of Right provisions, by identifying an income amount previously reported by the taxpayer that was required to be repaid during the current taxable year and the associated tax that was paid on that income. To claim this credit, the total amount repaid during the current tax year must be more than $3,000.
When the property tax credit is claimed by an individual who is a renter (tenant), the owner or lessor of the property completes this form to provide information to the tenant to determine what percentage of rental payments is attributable to property tax on the tenant's unit.
Form is used by either a surviving spouse, personal representative, or other person claiming a refund on behalf of a deceased taxpayer. This form must be completed and included with the deceased taxpayer's income tax return, when filed.
