TPT License Renewal: The Arizona Department of Revenue is advising businesses to renew their transaction privilege tax (TPT) licenses, which are due January 1, 2023, and penalties will be assessed for all renewals after January 31.

Request for Reduced Withholding to Designate for Tax Credits

The employee completes this form to notify his/her employer of the amount by which he/she wishes his/her withholding be reduced and the amount(s) sent as a quarterly payment to qualifying charitable organizations.

Withholding Transmittal of Wage and Tax Statements

This form is used by employers that electronically file Form(s) A1-R or A1-APR to transmit wage and tax statements to the department.

Arizona Withholding Reconciliation Tax Return

Employers that complete and file Form A1-QRT complete this form to reconcile their withholding deposits made with the Department of Revenue.

Quarterly Payment of Reduced Withholding for Tax Credits

Employers use this form to report contributions made by their employees to qualifying charitable organizations.  This form is completed quarterly and forwarded to the qualifying charitable organization(s), not the department.

Employer's Election to Not Withhold Arizona Taxes in December (Includes Instructions)

Used by employers to notify the Department of Revenue of the their intent to NOT withhold Arizona income taxes during the month of December.  Form A1-E must be filed with ADOR by July 1 of the year to which it will apply.
