Requesting Copies of Tax Documents?

The Arizona Department of Revenue will move to a flat fee pricing structure when taxpayers are requesting copies of tax documents; one price for certified copies and another for uncertified copies. Form 450 Request for Copies of Tax Documents will be posted to the Forms Page of the website in early December with the change in pricing taking effect on December 17, 2012.
