Affidavit of Nonresidency by a Resident of Another U.S. State Purchase of Motor Vehicle in Arizona

 

Form 5011 is a form the seller presents to the purchaser to complete.  The form is an agreement between the seller and the purchaser.  Do not send any copies of this form to the Arizona Department of Revenue; the purchaser and seller keeps the form for records.  

Form 5011 is a questionnaire for the seller to determine if the purchaser qualifies as an out-of-state resident. 

Arizona Resale Certificate

The purpose of the Certificate is to document the purchase of tangible personal property for resale in the purchaser’s regular course of business. It is to be filled out completely by the purchaser and furnished to the vendor. The vendor shall retain this Certificate for single transactions or for specified periods as indicated below. This Certificate shall be obtained from the purchaser at the time of the sale.

 

The form does not need to be sent to the Arizona Department of Revenue.
