Identity Theft Affidavit
This form is used to apply for transaction privilege tax, use tax, and Employer Withholding and Unemployment Insurance. The Application is called “Joint” because it is used by both the Arizona Department of Revenue and Economic Security.
Form used by Native American Veterans to request a refund of Arizona income tax withheld from their active duty military pay. Native American Veterans applicants must show they served in the military from 1977 – 2005 while they were a resident of Indian tribal land in order to receive a refund under the Native American Income Tax Settlement Fund.
Form used to substantiate the Native American Veteran was a resident of Indian tribal land during the period any Arizona income tax was withheld from their active duty military pay.

A victim of identity theft may use this form to request a redacted copy (one with some information blacked out) of a fraudulent return that was filed using the identity theft victim’s name and social security number. Although some information will be redacted, the remaining information will allow you to determine what information the identity thief may have about you.
