Ways to Obtain Forms

The Arizona Department of Revenue (ADOR) has free tax forms and provides several options to obtain the forms.  Please note: ADOR does not automatically mail paper tax booklets, unless you filed Form 140PTC in the prior year.

If you need forms, here are four easy ways:

  1. On our website. You can access forms on the website anytime here.
     
  2. In our lobbies. In each of our three lobbies, you will find tax booklets for your convenience to take home and use.
     
  3. Free Tax Return Preparation Services. There are hundreds of taxpayer assistance locations to receive help and pick up tax forms.

Volunteer Income Tax Assistance (VITA) and Tax Counseling for the Elderly (TCE) sites are located throughout Arizona. To locate the nearest VITA or TCE site near you, use the VITA Locator Tool by visiting www.irs.gov/individuals/free-tax-return-preparation-for-you-by-volunteers.

Additionally, AARP Tax Aide offers free assistance for electronically filing federal and Arizona state returns.

  1. In Your Community. During the tax filing season, many libraries and post offices offer tax booklets to make copies from. The booklets are strictly for copying and not to be taken from the premise. Library staff can assist with locating, copying and printing tax forms but are not able to give advice regarding forms.
